Patient Information
Prescriptions
Medication refills should be requested through your pharmacy. They will then fax us the request. We require 48 hours notice for routine refill requests. Please be aware that most requests for pain medication and antibiotics require an appointment. We require at least a yearly visit for stable medication problems, but more frequent visits may be required as determined by your provider.
Please call for prescription refills during regular office hours. No refills will be given after hours, weekends or holidays.
Financial Information
Payment of all co-insurances, co-payments, deductibles, and past due balances are due at the time of service. Additionally, if you do not have insurance, payment is required at the time of service.
We accept cash, checks, debit, and all major credit cards.
Your Medical Records
Information regarding medical records can be obtained by calling our main office numbers.
Medical records are kept confidential. We will not release any of your medical information without first obtaining a signed authorization from you.
